Intersectoral documentation and communication – success and failure stories in the US
Health literacy is at the heart of promoting health and wellness. The healthcare community should encourage strong documentation practices, consumer access and communication while protecting privacy and confidentiality. Patients and families are leading care decisions. Promoting health information "where and when they need it" provides a solid foundation to build consumer trust, provide continuity and support interoperability. This presentation focuses on how to build a community of literacy and engagement by becoming partners in care with the consumer.
